Last year, a coroner ruled that there were ‘gross failures’ in hospital care which led to her death.

North Bristol NHS Trust Chief Medical Officer has said: “We have fully accepted the omissions in Maddy’s care found by the coroner and continue to take action to address them.”
The statement concluded: “We are committed to making improvements to prevent tragic deaths like Maddy’s from happening in the future”
