* Holds a valid blue or orange CHAS card or Public Assistance (PA) card
* Are 50 to 80 years old, and have smoked ≥ 20 pack years, who are continuing to smoke or quit < 15 years ago
* OR are 50 to 75 years old with 1st degree family history (parent, child or sibling) of lung cancer for non-smokers
