3 things I never drink/ eat as a Nutritionist: 1) Soda (regular OR Diet). Adult women have approx 55% of our bodies made up of water, not soda. Therefore, this is the liquid our bodies need. Period. 2) processed/ deli meats: yes it’s protein BUT… these meats tend to be lower in protein than other kinds of meat because other animal parts are added. They are also very high in sodium and saturated fat. The relationship between the gut bacteria and systemic estrogen levels still remains unclear and research into the estrobolome is still in its infancy (even though it was first defined over ten years ago). There are bacteria in the gut that can either help eliminate excess estrogen or recycle it and send it back into our systemic circulation. There is early research looking at the interaction of the estrobolome and effects on endometriosis, menopause, estrogen positive breast cancer, chronic pelvic pain, infertility and PCOS but there is no peer-reviewed evidence or accepted standard yet for testing stool or blood to determine levels of an enzyme called Beta-glucuronidase activity in the gut (so beware of any service offering this). BUT we do know the things that are good for the gut microbiome and estrobolome: high fiber diet, fermented foods, being around nature and pets, avoiding unnecessary antibiotics, minimizing added sugar, alcohol…this is nutritional gynecology and I’m here for it! Protein & Fiber packed 10 second snack: This is one of my favorites. Here is what I didn’t explain in the video: Coconut oil: I will be talking a lot more about this in future issues of Ajenda, but as a Nutritionist and the daughter of a cardiologist, here is my bottom line on coconut oil: According to Harvard reviews, “in a meta-analysis of 16 clinical trials, coconut oil was found to increase both LDL and HDL cholesterol levels in participants, compared with nontropical vegetable oils (e.g., sunflower, canola, olive). Coconut oil increased total cholesterol by about 15 points, LDL by 10 points, and HDL by 4 points. Coconut oil also increased these values when compared with another tropical oil, palm oil: total cholesterol increased by about 25 points, LDL by 20 points, and HDL by 3 points. The analysis did not find that coconut oil versus other vegetable oils had any significant effect on body weight, waist circumference, or body fat percentage. The American Heart Association (AHA) issued a scientific advisory statement in 2017 to replace saturated fats (including coconut and other tropical oils) with unsaturated fats. “ (reference: Harvard Health). What I do like about Violife vegan cream cheese is that it is vegan and nut free, so I do occasionally use it. Smoked Salmon: there is a great local and very historic deli near me in NYC that has the best smoked salmon, but when I am in LA or Boston, I use packaged smoked salmon. There is no difference in the nutritional value. Many women are told that they have an ovarian cyst, but what does that actually mean? What questions should be asked/ what information is needed to make a plan for follow-up/ treatment? In gynecology, we don’t consider something to be a “cyst” unless it is 2.5cm or larger. Smaller than that, it’s considered a follicle. In general, 2.5-4cm is considered small, 4-7cm is considered medium size and 7cm is considered large. Most small cysts resolve spontaneously in pre-menopausal women. Medium to large size cysts are at risk for torsion, which is a twisting of the ovarian blood supply, and is considered a surgical emergency. Cysts can be simple or complex, and can also be seen in women with endometriosis. In early pregnancy, a corpus luteum cyst occurs which is a normal finding.
